hi, i have a nec 4230 laptop , works fine but it says cmos battery dead.
i`ve replaced it with a used battery , exact same voltage , but looks
slightly differant .
but it still says cmos battery dead , i`ve left it on / charging for a
couple days , to see if the cmos battery needs charging but still no go.

could the battery still be bad ?

The CMOS battery is a 3v Lithium "button cell" (watch battery). If you replaced the lithium cell with a used one, there's a very good chance that the replacement is as dead as the original

I have the same machine with the same issue - I removed the CMOS battery and measured it with a voltmeter and read 0.8v - IOW, nada! When you replace the battery you need to either get an exact replacent (pre-wired to the plug) or solder the leads to a new cell. If you do the latter you need to be careful to get the polarity right - read lead to the "+" side, and black lead to the other

Note that on most watch-battery type button cells, the "top" of the battery is the negative (-) pole and the shell of the battery is the positive (+) pole. (and is usually marked with a "+"

Once you get a good battery in there, that should fix you right up!
